By checking this, I agree to the terms outlined below.

I understand this is only an initial request and does not guarantee my donation request will be approved. If accepted I will be required to provide federal tax ID (and possibly a W-9) for the organization. I will not promote the Donation/Sponsorship until an official approval from Starbird has been received. Use of the Starbird logo or name on marketing materials must be approved by Starbird prior to distribution.

To be considered, please fill out this form at least two weeks in advance of your desired donation date. Once we receive your application, we will notify you are approved for a donation to confirm the date and provide the next steps.

Contest Rules
By submitting your email address, you are consenting to Starbird's marketing mailing list for future announcements and promotional opportunities. The winner will be contacted via email on July 3rd, if the winner declines the offer another winner will be chosen. The winner must pick up the party package on July 4th at one of our 4 Bay Area locations (San Francisco, Sunnyvale, San Jose, Foster City) from 10:30am - 10:00pm.
